Supportive Housing is the right choice for people who require access to 24 hour support and supervision and requiring some assistance managing with physical limitations, or ongoing health conditions such as dementia. Residents receive support and cueing with activities of daily living such as bathing, dressing, and medication reminders. If you think a loved one is a good fit for this option, contact the Supportive Housing residence you are interested in, to discuss living there.

When is it time to moved into a Personal Care Home

A Personal Care Home (PCH), also known as a "Nursing Home" or "long-term care home," provides personal care services to individuals who can no longer manage independently at home with family support and community services.

Eligibility will be determined by your local Regional Health Authority.
